Postoperative diagnosis: Right hamstring avulsion involving all three tendons, semitendinosus, semimembranosus and biceps femoris
off of the ischium with a complete avulsion hamstring tendons from the ischium with 5 cm retraction and scarring to the sciatic nerve

Doctors OP Notes:
A reverse L curvilinear incision was utilized to gain access to the hamstrings.
This was done utilizing part of the incicsion in the gluteal fold and vertical incision over the medial 1/3 of the posterior thigh.
The dissection was done meticulously down onto the subcutaneous tissue and fascia.
The posterior femoral cutaneous nerve was preserved.
The glueteal fold was then isolated and separated off of the hamstrings, the hamstrings were scareed to the area in addition to having thickened peritenon. Peritenon was incised.
A significant amount of serohemorrhagic fluid was sucked out of the sheath and then the tendons were noted to be completely avulsed.
A rongeur was then utilized to clean the base of the ischium down to the pelvis on either side of the ischium and ___ and into the area of the ischium. This provided excellent exposure along with using a retractor light.
The tendons were then freshened up using the rongeur and removing necrotic tissue from the ends of the tendons the tendons were then oriented and reapproximated using #2 FiberWire sutures x2 on 5.5 composite Bio-Corkscrew Arthrex anchors.
These were placed after first putting in a pilot hole tapping and then placing the anchors with an excellent bite into the ischium.
Sutures were then passed through the tendon using both a modified Kessler along with a Krakow stitch and then a sliding knot.
This brought the tendons back down to their footprint.
This was done after first placing all the sutures with eight limbs were tied. This was done tying each limb to its mate with a post using a sliding know and then tying the sutures to each other.
This provided an excellent stable repair.
Sutures were then cut.
The hip was brought out to approximately 30 degrees of extension of the knee with 30 degrees of flexion of the knee, without excessive tension on the repair itself.
All the scar tissue from around the tendon sheath was also freed up.
The sciatic nerve was visualized and was protected throughout the case and scar tissue was removed digitally from the tendon sheath and the sciatic nerve itself.
With this completed, the wound was then irrigated.
The tendon sheath was then reappoximated and closed using interruped 0 Vicryl sutures.
Subcutaneous tissue and fascia was reappoximated and closed using 2-0 Vicryl sutures and then the skin edges were reappoximated and closedusing staples. |

We have no power in my section of Attleboro MA. National Grid has put out a blanket statement that power may be out for "several days". So it looks like cold showers for me for a while... Oh, and my BF-1942 server is down, of course.
